Treatment Plan:

Plaquenil, minocycline, rifampin, dapsone, leucovorin, Malarone, hydrocortisone, DHEA, high doses of probiotics to prevent yeast infections and diarrhea, and (6S)-5-methyltetrahydrofolic acid with glucosamine salt.
